Where was The Joker in The Dark Knight Rises?

Christopher Nolan had made it quite clear that he had no intentions to mention The Joker in The Dark Knight Rises, but this isn't the same for the novel.
Christopher Nolan decided to not mention The Joker as a sign of respect, due to the tragic death of Heath Ledger after they finished filming The Dark Knight.
Fans had thought that Nolan may have used unused footage from The Dark Knight so that Joker could have a little cameo.

The movie doesn't make any references to The Joker, so it was up to us fans to just imagine what had happened to him in the last eight years. Well not anymore, because the novel of the same name written by Greg Cox has now revealed where The Clown Prince of Crime was placed.

Here is an extract from the novel explaining where The Joker was during the chaos:

"Now that the Dent Act had made it all but impossible for the city’s criminals to cop an insanity plea, it (Blackgate Prison) had replaced Arkham Asylum as a preferred location for imprisoning both convicted and suspected felons. The worst of the worst were sent here, except for the Joker, who, rumor had it, was locked away as Arkham’s sole remaining inmate. Or perhaps he had escaped. Nobody was really sure. Not even Selina."


It is unknown whether or not that this was Nolan's original idea before the death of Ledger, but I think that a little cameo for Joker would have been great.
Maybe we could have seen The Joker in a scene where he's just sitting in Arkham giggling.
If Ledger hadn't passed away, the movie could have been completely different!
